La giovane Nell è sopravvissuta ai fatti raccontati nel film precedente, L'ultimo esorcismo, ma è comprensibilmente traumatizzata. Dopo un ricovero in ospedale, viene accolta alla Devereux Hall, a New Orleans, una sorta di rifugio per ragazze problematiche a cura del dottor Frank Merle. Questi le assicura che nessuno è sopravvissuto oltre a lei, ma Nell nega: anche il demone Abalam ce l'ha fatta. Con il passare dei mesi e delle cure, Nell si convince però che Abalam era solo il frutto della sua immaginazione. Fa amicizia con le altre ragazze di Devereux Hall e sviluppa una simpatia per Chris, un collega del posto di lavoro che le è stato trovato per integrare il suo cammino verso la normalità. Però la quiete è di breve durata e i fantasmi del passato tornano a turbare la povera Nell. E non solo lei.
